I acknowledge that the nature of the exercise class I am about to undertake has

been fully explained, and all questions I had were answered to my full satisfaction. I

will inform the instructor of any changes to the above should they arise. Whilst I am

aware that all care will be taken to keep the class safe and enjoyable, I participate in

this exercise session of my own free will and understand that as with any exercise

programme there is a risk of injury.
