I hereby declare that I am in a fit state of health to undertake a physical exercise at Eclipse Gym Kyneton.  I acknowledge that I have provided all relevant medical details to the instructor before participating in a gym  class or program with Eclipse Gym Kyneton.

I hereby agree to accept sole responsibility for any physical condition that may arise from my participation and I understand that the exercise program I undertake is at my own risk.
